Do Massimo and Laura end up together?
The book ends with Laura choosing to be with Nacho, but that is after Massimo holds her against her will, drugs her and tries to get her pregnant. It's not really a surprise she escaped with Nacho in that scenario. The epilogue shows they have a daughter together, while Massimo has given up and agreed to a divorce.
But The Next 365 Days leaves the ending ambiguous. Laura returns to Massimo and they share a moment on the beach. However, the movie does not tell us definitively whether or not Laura and Massimo got back together or if she went back to Nacho.
Does Laura choose Nacho or Massimo? In the book (“Kolejne 365 dni”), Laura chooses Nacho.
After the shooting, an uneasy truce is agreed between the two sides. Massimo is angry after the death of his brother and tells his rivals to leave. Adriano is dead while Laura is alive. In fact, Laura has fully recovered from her gunshot wound last movie, and beyond a few mentions it's never brought up again.
While the movie deviated a bit from the second book (especially with the character of Nacho), it ended in the same way with Laura shot. However, a big difference was that she was still pregnant with Massimo's baby.
Massimo is still a mafia boss and still guilty of kidnapping, coercion, and a host of other criminal charges, but viewers can rest assured that he is no cheater.
Alas no. With 365 Days 4 not even confirmed by Netflix, filming hasn't started and therefore footage will be a long time away. Even the third movie only got its trailer weeks before its release. If you need some Laura and Massimo sexy times, you can rewatch the entire trilogy right now on Netflix.

By the end of The Next 365 Days, Laura doesn't choose either Nacho or Massimo. When she arrives back in Sicily, it's revealed that Nacho has snuck onto the island as well and he drives her back to Massimo's, confirming during the car journey that he is deeply in love with her.
The second and third installments of 365 Days have already begun shooting in Poland and Italy, with a worldwide release on Netflix scheduled for 2022.

365 Days: This Day spoilers: Did Massimo cheat on Laura? We later find out that Massimo did not cheat on Laura. It was actually his evil twin brother Adriano (also played by Morrone) who is scheming with Anna and…get this, Nacho's father!
It turns out that Anna was actually having sex with Massimo's evil twin brother, Adriano. Massimo never bothered to tell his wife about his crazy evil twin (as you do) and as such, it was all one big misunderstanding.
Throughout The Next 365 Days, Laura basically goes back and forth between Massimo and Nacho. While Laura is still married to Massimo, she also has serious feelings for Nacho.
